The famous 28-day cycle is an average, not a rule: in one study of healthy, regularly cycling women, 95% of cycles fell between 22 and 36 days. If your cycle sits in that broad range and is fairly predictable for you, it is very likely within normal limits.

How do you count the length of your cycle?

Day one is the first day of full menstrual bleeding (not light spotting beforehand). Your cycle ends on the day before your next period begins. So if one period starts on the 1st and the next on the 30th, your cycle was 29 days long.

Cycles are often described in two main parts:

The follicular phase: from the first day of your period until ovulation, when an egg is released.

The luteal phase: from ovulation until the next period starts.

Research on healthy women suggests the follicular phase contributes most to the differences in cycle length from one woman to another, and from one month to the next (Fehring et al., 2006).

Is 28 days really the norm?

Not really. It is a convenient figure that has become a cultural shorthand, but real-life data tell a more varied story.

In a study of 141 healthy women who tracked up to 13 cycles each, the mean cycle length was 28.9 days, with 95% of cycles between 22 and 36 days (Fehring et al., 2006).

Another study, in 167 women trying to conceive, found an average cycle of 27.7 days, with ovulation around day 14.7 on average, give or take a couple of days (Cole et al., 2009).

In an analysis of data from more than 1.5 million app users worldwide, only about 16% of women had a median cycle length of exactly 28 days (Grieger and Norman, 2020).

In a long-running study of south Indian women, the mean cycle was 31.8 days, and in no age group did 28-day cycles occur in more than 9% of women (Jeyaseelan et al., 1992).

The message is reassuring: a cycle of 26, 30 or 33 days is not automatically a problem. What matters more is what is typical for you, and whether it changes noticeably.

Why does cycle length vary from month to month?

Even in women with regular cycles, the length can shift. In the Marquette study, more than 7 days of variation within the same woman’s cycles was seen in 42.5% of participants (Fehring et al., 2006). A few things are linked with this variability.

Ovulation timing

Ovulation does not always happen on day 14. Only 25% of the women in one study had their entire fertile window fall between days 10 and 17 of the cycle, which is why predicting ovulation by the calendar alone can be unreliable (Fehring et al., 2006).

Body weight and body composition

Research in Sikkim, India found that a BMI above or below the WHO normal range was associated with more irregular cycles (Williams, 2006). A study of Austrian women also linked body type, especially the amount of subcutaneous fat, with both cycle length and regularity (Kirchengast, 1994). This does not mean there is a “right” body for a regular cycle, only that body composition is one of several influences.

Season and environment

In the same Sikkim study, the number of irregular cycles varied by season (Williams, 2006). Many women in Malta notice that long, hot summers, disrupted sleep or travel back to a home country seem to nudge their cycle. The research does not prove those specific triggers, but it does suggest the environment can play a part.

Stress, sleep and routine

These are commonly mentioned by clinicians as things that may shift ovulation, although the studies provided here did not measure them directly. A busy few weeks or a change in routine can plausibly make one cycle longer or shorter than usual.

Other factors

One older study found that women who were celibate had more variable cycles than women who were regularly sexually active (Burleson et al., 1991). The reasons are still debated, and it is not something to worry about or act on.

How does cycle length change through life?

Cycles tend to be least predictable at the two ends of the reproductive years. In the south Indian cohort, variability was high in girls under 19, settled between ages 25 and 39, and then increased again from 40 onwards (Jeyaseelan et al., 1992). That fits what many women in their forties describe: cycles that become a little shorter, longer or less predictable as perimenopause approaches.

If you are between 25 and 39, your cycle may be at its most stable. That does not mean it has to be perfectly regular, but sudden changes are worth noticing.

When should you speak to a GP or pharmacist?

Occasional variation is normal, but it is sensible to get advice if you notice any of the following:

Your cycles are regularly shorter than about 21 days or longer than about 35 days.

Your periods stop for three months or more and you are not pregnant, breastfeeding or on hormonal contraception.

Your cycle suddenly changes and stays different.

Bleeding is very heavy (for example, soaking through protection every hour or two), lasts much longer than usual, or comes between periods or after sex.

You have severe pain, or you are trying to conceive and cycles are very irregular.

Irregular cycles can have many causes, including thyroid conditions, polycystic ovary syndrome, and changes linked to perimenopause. A GP can check these properly. A pharmacist can also help with questions about contraception or over-the-counter pain relief.

How can you track your own pattern?

A few simple habits give you a clearer picture than any average:

Note the first day of each period in a diary or app for at least three to six cycles.

Record flow, pain, mood and sleep if you like, as these details can help a GP spot patterns.

Look at your range (for example, 27 to 31 days) rather than expecting the same number every time.

Remember that an app’s predictions are estimates based on your past cycles, and not a guarantee of when you will ovulate or bleed.

Frequently asked questions

Is a 35-day cycle normal?

It can be. Cycles of up to 36 days were seen in healthy, regularly cycling women (Fehring et al., 2006), and average cycle length is longer in some populations (Jeyaseelan et al., 1992). If a 35-day cycle is your usual pattern, it is probably normal for you. If it is new, or you are unsure, ask your GP.

Is a 21-day cycle normal?

A cycle of 21 to 22 days sits at the short end of the normal range. If it happens now and then, it is usually not a concern. If your cycles are consistently this short, or you also have heavy bleeding, it is worth discussing with your GP.

Can my cycle length change from month to month?

Yes. Variation of several days between cycles is common even in healthy women, and the time before ovulation accounts for most of it (Fehring et al., 2006).

Does a cycle that is not 28 days mean I am not ovulating?

Not necessarily. Ovulation happens at different points in different women, and in studies the timing varied widely even in women with regular cycles (Cole et al., 2009; Fehring et al., 2006). If you are trying to conceive and are unsure whether you are ovulating, a GP can advise on testing.

References

Variability in the phases of the menstrual cycle (2006)

The normal variabilities of the menstrual cycle (2009)

Menstrual cycle length and patterns in a global cohort of women using a mobile phone app (2020)

Pattern of menstrual cycle length in south Indian women (1992)

Menstrual cycle characteristics and predictability of ovulation of Bhutia women in Sikkim, India (2006)

Intercorrelations between menstrual cycle patterns and body dimensions in Austrian women (1994)

Heterosexual activity and cycle length variability: effect of gynecological maturity (1991)

This article is for general information and does not replace medical advice.
